บทคัดย่อ
รายงานผู้ป่วยฉบับนี้มีวัตถุประสงค์เพื่อนำเสนอแนวทางการจัดการปุ่มกระดูกขนาดใหญ่หลายตำแหน่งในผู้ป่วยที่ต้องเข้าสู่ภาวะไร้ฟันสมบูรณ์เพื่อรับการบูรณะด้วยฟันเทียมทั้งปาก โดยใช้เทคนิคการผ่าตัดแบบแบ่งระยะผู้ป่วยที่นำเสนอเป็นชายไทย อายุ 75 ปี ที่มีภาวะสันเหงือกไร้ฟันบางส่วน ตรวจพบปุ่มกระดูกขนาดใหญ่หลายตำแหน่ง ประกอบด้วยปุ่มกระดูกด้านแก้มของขากรรไกรบนทั้งสองข้าง ทอดยาวตั้งแต่ตำแหน่งฟันหน้าจนถึงฟันกรามซี่ที่สอง และปุ่มกระดูกด้านลิ้นในขากรรไกรล่างทั้งสองข้างที่มีลักษณะเป็นก้อนมน ซึ่งล้วนเป็นอุปสรรคต่อทิศทางการใส่ฟันเทียม ผู้ป่วยมีความประสงค์ที่จะใส่ฟันเทียมเพื่อฟื้นฟูประสิทธิภาพการบดเคี้ยว
แผนการรักษาประกอบด้วยการผ่าตัดตกแต่งสันเหงือกและกำจัดปุ่มกระดูกในขากรรไกรบนและล่างทั้งสองข้าง ร่วมกับการตัดเนื้อเยื่อยึดเกาะริมฝีปากบน โดยใช้แนวทางการผ่าตัดแบบแบ่งระยะออกเป็น 4 ครั้ง เพื่อลดความรุนแรงของอาการภายหลังการผ่าตัด ลดความเสี่ยงของภาวะแทรกซ้อน และส่งเสริมการหายของแผลแบบปฐมภูมิ
การติดตามผลที่ระยะ 1 เดือนและ 3 เดือน พบว่ามีการสมานตัวที่ดี สันเหงือกมีความเรียบมนและมีความกว้างเพียงพอต่อการรองรับฐานฟันเทียม ภายหลังการบูรณะด้วยฟันเทียมทั้งปาก ผลการประเมินคุณภาพชีวิตในมิติสุขภาพช่องปากของผู้ป่วยปรากฏแนวโน้มที่ดีขึ้น ผลลัพธ์ดังกล่าวสะท้อนให้เห็นว่า การวางแผนการรักษาอย่างเป็นระบบและการใช้เทคนิคการผ่าตัดแบบแบ่งระยะสำหรับปุ่มกระดูกขนาดใหญ่หลายตำแหน่ง มีส่วนสำคัญต่อความสำเร็จในการฟื้นฟูระบบบดเคี้ยวของผู้ป่วย
