The Management of Multiple Large Oral Tori and Exostoses Prior to Complete Dentures Rehabilitation: A Case Report

Abstract

This case report aims to present a clinical management approach for multiple large oral tori and exostoses in a patient transitioning to a fully edentulous state for complete denture rehabilitation, employing a staged surgical approach. The case involves a 75-year-old  Thai male patient with partial edentulism who exhibited multiple large oral tori and exostoses. The lesions included bilateral maxillary buccal exostoses extending from the anterior region to the second molars, and bilateral lobulated mandibular tori, all of which obstructed the prospective path of insertion for dentures. The patient sought prosthetic rehabilitation to restore masticatory function.


Treatment consisted of alveoloplasty and torectomy in both the maxilla and mandible, combined with an upper labial frenectomy. A staged surgical approach was performed in four separate phases to minimize the severity of postoperative symptoms, reduce the risk of complications, and promote primary wound healing.


Follow-up examinations at one and three months revealed excellent wound healing. The alveolar ridges were smooth, well-contoured, and sufficiently wide to support the denture bases. Following rehabilitation with complete dentures, the patient's oral health-related quality of life showed a positive trend of improvement. These findings demonstrate that systematic treatment planning and the application of a staged surgical technique for multiple large oral tori and exostoses play a crucial role in the successful masticatory rehabilitation of the patient.
