การรักษาฟันฝังร่วมกับควักถุงน้ำเดนทิเจอรัสที่สัมพันธ์กับโพรงอากาศขากรรไกรบน : รายงานผู้ป่วย

บทคัดย่อ
รายงานผู้ป่วยนี้มีวัตถุประสงค์เพื่อนำเสนอการวินิจฉัยและรักษาฟันฝังร่วมกับถุงน้ำเดนทิเจอรัสที่มีความสัมพันธ์กับโพรงอากาศขากรรไกรบน ซึ่งเป็นถุงน้ำพัฒนาการที่เกิดจากฟัน มักไม่มีอาการและตรวจพบโดยบังเอิญจากภาพรังสี โดยทั่วไปมักพบในฟันคุดขากรรไกรล่างมากกว่า ในรายงานนี้เป็นผู้ป่วยหญิงอายุ 37 ปี มาพบทันตแพทย์จากอาการบวมบริเวณฟันกรามน้อยบนที่ไม่ขึ้นในช่องปาก ไม่มีอาการปวดหรือการขยายตัวของรอยโรค ผลการตรวจทางคลินิก ภาพรังสี และพยาธิวิทยายืนยันว่าเป็นถุงน้ำเดนทิเจอรัสเกี่ยวข้องกับฟันฝังและลุกลามสู่โพรงอากาศขากรรไกรบน ซึ่งเป็นลักษณะที่พบได้น้อยทางระบาดวิทยา
การรักษาประกอบด้วยการผ่าตัดควักถุงน้ำออกทั้งหมดร่วมกับนำฟันฝังที่เป็นสาเหตุออก เพื่อกำจัดรอยโรคและลดโอกาสการกลับเป็นซ้ำ ระหว่างการผ่าตัดพบรูทะลุช่องปาก-โพรงอากาศขากรรไกรบน จึงทำการปิดรูทะลุโดยใช้เนื้อเยื่อกระพุ้งแก้มเพื่อป้องกันการรั่วไหลของเชื้อโรคหรือของเหลว การผ่าตัดมีความซับซ้อนและต้องได้รับการดูแลหลังผ่าตัดจากผู้ป่วย การสื่อสารและการให้ข้อมูลที่ชัดเจนจึงมีความสำคัญเพื่อให้เกิดความร่วมมือและลดภาวะแทรกซ้อนที่อาจเกิดขึ้น
ผลการรักษาพบว่าผู้ป่วยกระดูกและเนื้อเยื่ออ่อนหายเป็นอย่างดี ไม่พบการติดเชื้อ รูทะลุช่องปาก-โพรงอากาศขากรรไกรบนปิดสนิท ฟันข้างเคียงไม่มีความผิดปกติ ผลทางพยาธิวิทยาพบว่าเป็นถุงน้ำ ไม่พบเซลล์ที่ผิดปกติ อย่างไรก็ตามพบว่าร่องระหว่างเหงือกและกระพุ้งแก้มมีลักษณะตื้น ซึ่งอาจมีผลต่อการบูรณะฟันในอนาคต การติดตามผลเป็นระยะเวลา 3 เดือนไม่พบสัญญาณการกลับเป็นซ้ำของถุงน้ำ การรักษานี้สะท้อนความสำคัญของการวินิจฉัยที่ครบถ้วน การวางแผนการรักษาที่เหมาะสม และการติดตามผลการรักษาอย่างต่อเนื่องเพื่อให้ผลลัพธ์ที่ดี ลดภาวะ แทรกซ้อนระยะยาวของผู้ป่วย
