Enucleation of Dentigerous Cyst Associated with embedded tooth and maxillary sinus : A case report

Abstract

This case report aims to present the diagnosis and surgical management of a dentigerous cyst associated with an embedded maxillary premolar extending into the maxillary sinus-an uncommon presentation. Dentigerous cysts are developmental odontogenic lesions that are frequently asymptomatic and often detected incidentally on radiographic examination. A 37-year-old female presented with localized swelling in the maxillary premolar region, where the tooth had failed to erupt. She reported no pain or progressive enlargement. Clinical evaluation, radiographic findings, and histopathological examination confirmed a  cyst.


Treatment consisted of complete enucleation of the cyst, extraction of the associated embedded tooth, and closure of an intraoperatively detected oroantral communication using a buccal mucosal flap. Comprehensive patient communication and adherence to postoperative instructions were emphasized to reduce complications.


Postoperative healing was uneventful, with bone and soft-tissue recovery and no evidence of infection or sinus complications. The histopathological findings confirmed a cystic lesion without cellular atypia. A shallow buccal vestibule developed, which may affect future prosthetic or implant rehabilitation. At the three-month follow-up, no recurrence was observed. This case highlights the importance of accurate diagnosis, appropriate surgical planning, and long-term follow-up to ensure optimal outcomes in maxillary dentigerous cysts
