Development of a Nursing Care Model for the Treatment and Rehabilitation of Individuals with Substance Use Disorders at Nuea Khlong Hospital, Krabi Province

Abstract

Purpose: To develop and evaluate the effectiveness of a nursing model for the treatment and rehabilitation of drug users and addicts at Nuea Khlong Hospital, Krabi Province. The study aims to enhance readiness for behavioral change, increase internal motivation in developing drug cessation behaviors, and reduce the rate of drug relapse.

Methodology: This research and development (R&D) study was conducted in three phases: 1) Situational analysis of the treatment system through questionnaires (n = 135) and in-depth interviews (n = 15). 2) Development and validation of the nursing model. 3) Implementation and testing with 40 drug users and addicts. The research utilized 7 instruments, including a general information form, semi-structured interview guidelines, and assessments for: readiness for behavioral change, intrinsic motivation, drug cessation behavior, relapse follow-up, and satisfaction. The instruments demonstrated a Content Validity Index (IOC) of 0.90–1.00 and Cronbach’s alpha coefficient (α) of 0.82–0.91. Data were analyzed using descriptive statistics, paired t-tests, and content analysis.

Findings: Phase 1: The majority of the sample were working-age males. The treatment system demonstrated strengths in structure and accessibility (Mean = 4.05–4.39). However, limitations were identified regarding intrinsic motivation, family and community participation, and psycho-spiritual care (Mean = 2.36–2.48). Phase 2: A 4-step holistic nursing model was developed, consisting of: (1) individual assessment, (2) psycho-social-spiritual therapy, (3) motivation and risk management skill enhancement, and (4) follow-up to prevent relapse. Phase 3: After 30 days of program implementation, the participants showed statistically significant increases in readiness for behavioral change, intrinsic motivation, and drug cessation behavior (p<0.01). The relapse rate decreased to 10%, and overall satisfaction was at a high level.

Applications of this study: This nursing model can be applied in similar healthcare settings to promote behavioral change, strengthen intrinsic motivation, and reduce relapse through integrated bio-psycho-social-spiritual care.
